Kidney Stone
A kidney stone is a hard mineral and crystalline material formed within the kidney or urinary tract. Kidney stones are sometimes called renal calculi. Formation of crystal aggregates in the urinary tract results in kidney stones, the clinical condition referred to as nephrolithiasis. Kidney stones may produce no symptoms or may be associated with one or several of the following: flank pain, gross or microscopic hematuria, obstruction of one or both kidneys and urinary infections. The stones are usually formed by one of four substances: (1) calcium, (2) uric acid, (3) magnesium ammonium phosphates, or (4) cysteine. Occasionally, calcium salts and uric acid will be present in the same stone.
Cause: Kidney stones form when there is decrease in urine volume or an excess of stone-forming substances in the urine. The most common type of kidney stone contains calcium in combination with either oxalate or phosphate. Other chemical compounds that can form stones in the urinary tract include uric acid and the amino acid cystine.
A number of conditions lead to kidney stones like in gout where increased amount of uric acid present in urine; hypercalciuria where too much calcium is absorbed from food and excreted in urine, where it may form calcium phosphate or calcium oxalate stones; other conditions like hyperparathyroidism, kidney diseases like renal tubular acidosis and some inherited metabolic conditions including cystinuria and hyperoxaluria; medications like diuretics, calcium containing antacids etc
Role of Noni in Kidney Stone

Ureteral stones cause obstruction of urine transport. A number of dynamic changes, thought to be modulated by several vascoactive substances, occur with ureteral obstruction. Animal experimentation indicates that renal blood flow (RBF) increases immediately following obstruction, and decreases thereafter. Collecting system pressure rises during the first 5 hours after obstruction and then it subsequently decreases. The initial increase in RBF is mediated, at least partially, through prostaglandin and nitric oxide.
In vitro studies of human ureter demonstrated that prostaglandins increase ureteral smooth muscle contractility. Nonsteroidal anti-inflammatory drug (NSAID) agents that inhibit cyclooxygenase (COX) enzyme are used extensively in the management of patients with renal colic. Their benefits are based on various mechanisms including reduction in RBF that decreases pressure in collecting system, ureteral smooth muscle relaxation, and a decrease in stone-induced ureteral edema. The latter 2 (ureteral smooth muscle relaxation and decrease in stone-induced ureteral edema) effects of NSAIDs are hypothesized to facilitate stone passage as well. Two randomized controlled trials have assessed ability of NSAIDs to facilitate stone passage. The study demonstrated that patients receiving any nonselective COX inhibitor did not increase stone expulsion rate compared to placebo, but significantly decreased pain and hospital admissions.
The aforementioned studies suggest that nonselective COX inhibitors are not effective in facilitating stone passage. One research group found that the COX-2 protein and its mRNA are expressed to a greater degree in obstructed human ureter as compared to normal human ureter. Furthermore, this group demonstrated that a selective COX-2 inhibitor reduced the contractility of both human and swine ureter. Thus it can be hypothesized selective COX-2 inhibitor may facilitate stone passage by its property of ureteral smooth muscle relaxation. Noni is a selective COX-2 inhibitor. Noni can help a lot in the management of patient with renal colic. From the above study of selective COX-2 inhibitor, impact on stone passage can put a ray of hope of those renal stone patient if taking Noni.
