Piles
Piles or haemorrhoids, are areas in the anal canal where the tissue, which contains lots of blood vessels, has become swollen. Around half the population of INDIA suffer from piles at some point. They are common in all age groups from mid-teens onwards and particularly common in pregnant women.
Piles look like round pink swellings, the size of a pea or a grape. Sometimes they are of a blue colour.
Piles can be internal, inside the anus, or external, and be seen and felt on the outside of the anus. You can have internal and external piles at the same time.
Internal Piles
Internal piles are classified according to their position.
First-degree (or grade 1) piles are small swellings on the inside lining of the anal canal. They cannot be seen or felt from outside the anus.
Second-degree (or grade 2) piles are larger and stick out (or prolapse) from the anus when you go to the toilet, but return afterwards.
Third-degree (or grade 3) piles are similar, but hang out from the anus and only return inside when pushed back with a finger.
Fourth-degree (or grade 4) piles permanently hang down from the anus and you cannot push them back inside.
External Piles
An external haemorrhoids (perianal haematoma) is a small lump that develops on the outside edge of the anus. They are less common than internal haemorrhoids.
There are conditions other than piles that can cause bleeding from the anus. If you notice bleeding for more than six weeks, then you should get medical advice.
Causes
The exact way piles form is controversial but it may be linked to excess pressure in the anus and lower rectum.
- Chronic constipation invariably is the major cause of piles as the need to strain usually arises because waste product of low fibre foods creates small hard stools. These injure the mucous lining of the arms as they pass through. They do not pass easily and nearly every act of defecation can put a damaging strain on the anus. Usually the bleeding from piles is a squirt or drip with passage of stools.
- A positive family history is often obtained and is related to congenital weakness of vein walls.
- Any condition which causes excessive straining such as chronic cough and bronchitis, obesity, lifting heavy weights, raises the intra abdominal pressure and the incidence of piles.
- Physiological haemorrhoids (piles) are also seen during pregnancy when the pressure of the growing baby and the uterus gives rise to piles. It should be noted that this is a reversible condition and should not be operated.
- Psychological cause like over anxiety and extra ordinary mental stress also cause piles
